Funding & Assistance

There are three different types of funding that you might use:

01.

Home Care Packages

(HCPs)

A Home Care Package (HCP) is a government-funded program designed to help older Australians receive personalised care and support services while remaining in their own homes. HCPs are available in four levels, from basic care needs (Level 1) to high care needs (Level 4), allowing individuals to receive the level of care that suits their requirements. The goal is to enable seniors to live independently for longer, with the appropriate support tailored to their specific needs.

02.

User Pays /

Self Funding

User pays allows you to pay for home care services directly, without relying on government funding. This option is ideal for individuals who need immediate care, are not eligible for a Home Care Package, or simply prefer to have more control over their care services.

With self-funded care, you choose the services you need and pay for them out of pocket. This approach offers flexibility in terms of the type and amount of care you receive, as well as the provider you choose. You can tailor your care plan to suit your preferences, ensuring that you receive the exact support you need, when you need it.

03.

Commonwealth Home

Support Program

The Commonwealth Home Support Programme (CHSP) is a government initiative designed to provide entry-level support services to older Australians who need a little extra help to stay living independently in their homes. The CHSP is ideal for those who need short-term or low-level assistance and is often used as a stepping stone before transitioning to a Home Care Package.

Navigating the Home Care Package Process with Live Easy

Understanding and accessing Home Care Packages (HCP) can be complex, but with Live Easy, the process becomes straightforward and supportive. Here’s how we can help you every step of the way:

01
Getting Assessed

The journey begins by contacting My Aged Care, the Australian Government’s gateway to aged care services. During your initial contact, My Aged Care will gather information about your current situation, health status, and immediate care needs. Depending on this information, you may be referred for an assessment by either the Aged Care Assessment Team (ACAT) for more comprehensive care or the Regional Assessment Service (RAS) for entry-level support.

How Live Easy Helps: We assist you through this initial contact process, especially if English isn’t your first language. We can help translate all the information into simple, understandable terms and guide you on what to expect during the assessment.

02
Understanding Your Assessment

An assessor, typically a healthcare professional, will visit you in your home, hospital, or a community setting to discuss your health, mobility, cognitive function, social needs, and current support networks. They will also consider your preferences and cultural background to determine what level of care you’re eligible for, which might include Home Care Packages for comprehensive in-home care.

How Live Easy Helps: While waiting for your package to become available, we can talk with you about your care needs and even start planning ahead. We can highlight what you might be entitled to, helping you make the most informed decisions possible.

03
Choosing Your Provider

Once you receive your letter of approval from My Aged Care, outlining the services you’re eligible for, it’s time to choose a provider. This is a crucial step where you decide who will manage and deliver your care services.

How Live Easy Helps: By choosing Live Easy, you have the benefit of a consistent, friendly face who has already guided you through the process now providing your services. We make sure everything aligns with your needs and preferences, so you feel comfortable and supported.

04
Planning Your Care

Together, we’ll work to develop a personalised care plan that suits your needs. This plan will outline the types of services you’ll receive, whether it’s help with daily activities, health services, or social support.

How Live Easy Helps: We’re dedicated to working with you to refine and perfect your level of care, all within a budget that you set. Our collaborative approach ensures your care evolves with you as your needs change over time.

05
