The Support at Home program is designed to ensure you achieve the best possible outcomes from your aged care services. Central to this approach is care management, where a dedicated care partner, Live Easy Buddy from Live Easy will assist you throughout your care journey.


What Exactly is Care Management?

Care management involves working closely with Live Easy Buddy, who will help manage, coordinate, and tailor your care based on your specific needs and goals. Here’s how we will assist you:


1. Care Planning
  • Identifying your personal aged care needs, goals, preferences, and existing supports.

  • Creating and regularly reviewing a tailored care plan with your active participation.

  • Reviewing your service agreements to ensure they meet your needs.


2. Service Coordination
  • Coordinating communication between you, your family, carers, and care workers.

  • Managing your budget effectively and ensuring proper oversight.

  • Assisting you in smoothly transitioning between different types of care or when returning home from hospital.


3. Monitoring, Review, and Evaluation
  • Regularly discussing and monitoring your care plan to adapt to your changing needs.

  • Conducting case conferences as required.

  • Evaluating care effectiveness, service quality, and progress toward achieving your goals.


4. Support and Education
  • Empowering you with the information and resources needed to make informed decisions.

  • Providing guidance on reablement strategies to enhance your independence.

  • Offering education on health promotion and navigating the aged care system.

  • Resolving issues or concerns swiftly and effectively.

  • Supporting you in providing feedback or addressing complaints to ensure your voice is heard.


How Does the Care Planning Process Work?

Live Easy will collaborate with you to create a comprehensive care plan tailored specifically to your individual needs. This care plan will be reviewed and adjusted whenever necessary, guided by your aged care assessment.


Budgeting for Your Care Management

For ongoing services, 10% of your Support at Home budget is allocated each quarter to Live Easy specifically for care management. If you have diverse or complex needs, additional intensive support funding may also be provided.


Who Qualifies for Extra Support?

Additional intensive care management support is available for individuals who:

  • Have been referred by the Care Finder program

  • Identify as Aboriginal or Torres Strait Islander

  • Are homeless or at risk of homelessness

  • Are care leavers

  • Are veterans eligible for the Veterans' Supplement
